Preoperativni i postoperativni patohistološki nalaz kod bolesnica sa hiperplazijom endometrijuma

Sažetak

Cilj rada bio je da se analiziraju i kompariraju patohistološki nalazi na kiretmanu i operativno odstranjenoj materici, proceni pouzdanost patohistološke dijagnoze na kiretmanu i utvrdi učestalost koegzistirajućeg karcinoma endometrijuma kod bolesnica sa patohistološkom dijagnozom hiperplazije endometrijuma na kiretmanu. Retrospektivno su analizirani i komparirani patohistološki nalazi na kiretmanu i operativno odstranjenoj materici kod 135 bolesnica sa inicijalnom dijagnozom hiperplazije endometrijuma. Ne postoje statistički značajne razlike između preoperativnog i postoperativnog patohistološkog nalaza bolesnica (p>0,05). Pouzdanost patohistološke dijagnoze hiperplazije endometrijuma na kiretmanu iznosi 82,2-89,6% i zavisi od tipa hiperplazije endometrijuma. Koegzistirajući karcinom endometrijuma je statistički značajno češći (p<0,001) kod bolesnica sa atipičnom hiperplazijom endometrijuma nego kod bolesnica sa hiperplazijom endometrijuma bez citološke atipije. Nasuprot simpleks atipičnoj hiperplaziji, kompleksna atipična hiperplazija je signifikantno češće (p<0,05) udružena sa koegzistirajućim karcinomom endometrijuma. Učestalost koegzistirajućeg karcinoma endometrijuma u operativno odstranjenoj materici kod bolesnica sa patohistološkim nalazom atipične hiperplazije na kiretmanu je relativno visoka (27,8%) i ovo treba imati u vidu pri planiranju terapije.
