Lokalna primena koncentrovanih trombocita i stepen oseointegracije dentalnih implantata od titana: histomorfometrijska analiza

Sažetak

Uvod/cilj: Potreba da se stvore što bolji uslovi koji utiču na koštano zarastanje nakon ugradnje implantata, uslovila je u poslednjih par godina primenu faktora rasta koji direktno ili indirektno regulišu rast ćelija i tkiva. Jednostavnost dobijanja koncentrovanih trombocita izdvojenih iz plazme bogate trombocitima (KT-PRP), dovela je do široke primene faktora rasta poreklom iz trombocita poput faktora rasta dobijenog iz trombocita (PDGF), transformišućeg faktora rasta (TGF-β), insulinu sličnog faktora rasta (IGF), vaskularnog endotelnog faktora rasta (VEGF) i mnogih drugih, sa ciljem da se postigne kontakt površine implantata sa kosti u što većem procentu, što obezbeđuje povećanu stabilnost implantata i brže funkcionalno opterećenje. Cilj rada je da se utvrdi da li primena KT-PRP metode obezbeđuje veću površinu kontakta između kosti i implantata. Metode. Studija je sprovedena na šest eksperimentnih pasa kod kojih su ugrađena 24 BCT implantata i to po četiri (dva sa leve strane uz primenu KT-PRP i dva sa desne strane bez primene KT-PRP). Životinje su žrtvovane (po dve) nakon 42, 70 i 98 dana. Zatim se pristupilo pripremanju preparata i histomorfometrijskoj analizi. Histomorfometrijski su analizirana po dva preparata koštanog ležišta 42, 70 i 98 dana nakon ugradnje implantata, pri čemu su izmereni kontakti kost-implantat u 16 zona za svaki analizirani implantat saglasno metodologiji. Rezultati. Rezultati dobijeni histomorfometrijskom analizom ukazuju na povećan procenat kontakta kost-implantat u slučaju kada su primenjeni KTPRP. Razlika kontakta kost-implantat između ove grupe i grupe sa klasičnom metodom ugradnje implantata naročito je izražena u prvih šest nedelja. Zaključak. Na osnovu ispitivanja stepena oseointegracije kod ugradnje dentalnih implantata i dobijenih rezultata može se dati prednost primeni KT-PRP metode, jer je stepen oseointegracije dentalnih implantata veći kada su kod ugradnje primeni ova metoda.
