Klinička efikasnost ampicilina u lečenju akutnih dentogenih apscesa

Sažetak

Uvod/Cilj. Stavovi o izboru antibiotika i dužini njihove primene u terapiji akutnih dentogenih apscesa kontroverzni su. Cilj ove studije bio je da se utvrdi klinička efikasnost peroralne primene ampicilina u lečenju akutnih dentogenih apscesa, kao i da se ispita antimikrobna osetljivost izolovanih sojeva bakterija na ampicilin u ranoj fazi razvoja apscesa. Metode. U istraživanje je bilo uključeno 60 ispitanika sa dentogenim apscesima koji su hirurški lečeni (ekstrakcija zuba i/ili incizija apscesa) i svrstani u dve grupe, ampicilinsku i hiruršku grupu (bez primene antibiotika). Rezultati. U grupi ispitanika koji su dobijali ampicilin, lečenje je trajalo u proseku 4,67 dana, a u hirurškoj grupi 6,17 dana. Kod 60 bakterioloških nalaza izolovano je 78 različitih bakterijskih sojeva. Najzastupljenije bile su fakultativno anaerobne Gram-pozitivne bakterije (68/78), a predominantno zastupljena bakterija bila je Streptococcus viridans (43/78). Ukupna osetljivost izolovanih bakterija na ampicilin iznosila je 70,5%. Zaključak. Peroralna primena ampicilina u ranoj fazi razvoja apscesa nakon primene hirurškog metoda lečenja, statistički značajno skraćuje trajanje kliničkih simptoma akutne dentogene infekcije u odnosu na tretman samo hirurškim metodom. Izolovani bakterijski sojevi, u ranoj fazi razvoja apscesa, pokazuju visoku osetljivost na ampicilin.
