Modaliteti lečenja vezikoureternog refluksa kod dece

Sažetak

Uvod/Cilj. Vezikoureterni refluks aktuelan je problem u dečjoj urološkoj praksi. Optimalni modalitet njegovog lečenja i dalje je diskutabilan. Cilj studije bio je analiza kliničkog materijala bolesnika sa dijagnozom vezikoureternog refluksa. Posebno, razmatrani su rezultati endoskopskog tretmana instilacijom Defluks paste. Metode. Retrospektivnom studijom bili su obuhvaćeni bolesnici lečeni zbog vezikoureternog refluksa tokom petogodišnjeg perioda. Analizirani su uzrast, pol, stepen, lokalizacija refluksa, prisustvo urinarnih infekcija, udruženih anomalija i komplikacija, metode lečenja i uspešnost endoskopske korekcije instilacijom Defluks paste. Obrada podataka izvršena je standardnim statističkim metodama. Rezultati. Studijom je bilo obuhvaćeno 167 bolesnika (101 devojčica i 66 dečaka), prosečnog uzrasta 4,6 godina, sa dijagnozom vezikoureternog refluksa. Refluks je utvrđen jednostrano kod 61,7%, obostrano kod 38,3% bolesnika. Urinarne infekcije imalo je 78,4%, refluksnu nefropatiju 38,3%, hipertenziju 3,0%, hroničnu bubrežnu insuficijenciju 4,8%, udružene anomalije 39,5% bolesnika. Konzervativno je lečeno 80, a hirurški 74 bolesnika. Endoskopska instilacija Defluks paste sprovedena je kod 59 bolesnika. Od 79 tretiranih uretera, pet je imalo reluks I stepena, 11 II stepena, 23 III stepena, dva IV stepena i 13 V stepena. Instilacija Defluks paste izvršena je jednom kod 46, dva puta kod osam, tri puta kod pet bolesnika, nakon čega je odsustvo refluksa postignuto kod 42 (71,2%), poboljšanje kod 15 (25,4%) bolesnika, što zajedno čini 96,6% uspešnosti ove metode lečenja. Zaključak. Endoskopski tretman VUR instilacijom Defluks paste je minimalno invazivan metod lečenja vezikoureternog refluksa kod dece.
