Da li je ehokardiografski nalaz kalcifikacije mitralnog anulusa pokazatelj koronarne bolesti?

Sažetak

U ovom ispitivanju testirana je hipoteza da je kalcifikacija mitralnog anulusa, otkrivena transtoraksnom ehokardiografijom, pokazatelj koronarne bolesti. Kalcifikacija mitralnog anulusa se ehokardiografski definiše kao ograničena ehogena struktura iza zadnjeg listića mitralne valvule. Pedeset devet pacijenata sa kalcifikacijom mitralnog anulusa (28 žena i 31 muškarac srednje životne dobi 72±9 godina) upućenih na kateterizaciju srca i selektivnu koronarografiju zbog različitih indikacija upoređivano je sa pedesettri pacijenta (21 žena i 32 muškarca srednje starosti 63±6 godina) kontrolne grupe bez kalcifikacije mitralnog anulusa koji su upućeni na selektivnu koronarografiju iz istih razloga. Nalaz suženja glavnog stabla leve koronarne arterije koje je veće od 50% ili stenoze veće od 70% najmanje jedne epikardne koronarne arterije smatran je indikativnim za koronarnu bolest. U poređenju sa kontrolnom grupom u grupi bolesnika sa kalcifikacijom mitralnog anulusa postojala je značajno veća učestalost koronarne bolesti (86% prema 66%, p = 0.001), veća učestalost trosudovne koronarne bolesti (41% prema 20%, p = 0.001) i stenoze glavnog stabla leve koronarne arterije (10% prema 2%, p =0.001). Kalcifikacija mitralnog anulusa otkrivena transtoraksnom ehokardiografijom može poslužiti kao pokazatelj veće učestalosti i težine koronarne bolesti kod pacijenata upućenih na selektivnu koronarografiju.
