Trauma i operacija - medijatori inflamacije i lezije

Sažetak
Medijatori imaju ključnu ulogu u nastanku sindroma sistemskog inflamatornog odgovora (systemic inflammatory response syndrome - SIRS), sindroma multiple organske disfunkcije (multiple organ dysfunction syndrome - MODS), i sindroma multiple organske insuficijencije (multiple organ failure - MOF) vitalnih organa. Ovaj kratak revijalni članak prikazuje dosadašnja znanja o medijatorima inflamacija i lezija. Najpre se opisuju faktori koji nastaju tokom intenzivne traume (intraoperativni stres), odnosno supstancije koje se oslobađaju iz neutrofila i dovode do oštećenja tkiva, produkti aktivacije komplementa i selektini kao molekuli adherencije. Crevo u šoku ističe se kao značajan faktor tranzicije neinfektivnog u infektivni SIRS. Zatim se analiziraju medijatori koji se oslobađaju tokom sepse: citokini azot-monoksid, fosfolipaza A, faktor aktivacije trombocita i prokoagulantne supstancije. Oslobađanje medijatora uslovljeno je najpre intenzitetom traume, šoka ili sepse, a sekundarno aktivacijom različitih kaskada medijatora tokom posttraumatskih i postoperativnih komplikacija. Zbog toga su medijatori od izuzetnog značaja za procenu intenziteta lezije organa i ishoda lečenja.
