Polipragmazija (polifarmacija) kod pacijenata koji koriste usluge Hitne medicinske pomoći

Sažetak

Uvod. Polipragmazija je ozbiljan problem zbog svojih mnogobrojnih neželjenih posledica po zdravlje ljudi. Za polipragmaziju ne postoji jedinstvena i jasna definicija. Istraživači se, uglavnom opredeljuju za broj istovremeno upotrebljavanih lekova. U radu smo polifarmaciju definisali kao istovremeno korišćenje šest ili više lekova u zadnjih sedam dana, povremeno ili redovno. Cilj rada. Utvrditi broj istovremeno korišćenih lekova u polupragmaziji i povezanost sa polom i godinama starosti ispitanika. Metod. U beogradskoj Hitnoj medicinskoj pomoći jedan lekar je od 04.03.2008. do 29.03.2013. godine anketirao na terenu pacijente o lekovima koje su koristili redovno ili povremeno u poslednjih sedam dana, uključujući i minerale, vitamine, pomoćna lekovita sredstva i preparate u okviru tradicionalne medicine i te podatke unosio u Upitnik. Svi dobijeni podaci su unošeni u eksel tabelu i statistički obrađeni. Korišćen je programski paket SPSS 11.0 for Windows. Rezultati. Anketom je obuhvaćeno 1516 ispitanika oba pola (649 muškaraca i 867 žena), prosečne starosti 65,84±17,69 godina. Prosečan broj lekova koji su ispitanici istovremeno uzimali je 6,82±3,79(interval od 0 do 25) i nema statistički značajne reazlike u odnosu na pol. Šest i više lekova koristilo je 847 ispitanika (349 muškaraca i 498 žena). Prosečna starost za muškarce iznosila je 70,89±12,79 godina, a za žene=74,0±11,29. Najveći broj ispitanika (60,10%) bio je starosti 66- 85 godina. Najveći broj ispitanika je uzimao 6-10 lekova( 70,60%). Deset i više lekova je koristilo 39,79% ispitanika oba pola, a među onima koji su koristili ≥10 lekova čak 80,18% je starije od 65 godina. Muškarci su koristili u proseku 9,55±3,06 lekova, a žene 9,23±2,77 i nema statistički značajne razlike (p=0,136). Prosečan broj upotrebljavanih lekova povećavao se sa godinama i bio je najveći u grupi 66-75 godina. Statistički značajna razlika postojala je samo između mlađih i onih starijih od 65 godina. Od ukupnog broja ispitanika sa polipragmazijom, 74,97% su stariji od 65 godina. Oni su upotrebljavali više lekova u odnosu na mlađe i između njih postoji statistički značajna razlika (p=0,008). Zaključak. Više od polovine naših ispitanika je bilo u polipragmaziji i isto toliko u ekscesivnoj polipragmaziji. Polipragmazija je značajno povezana sa starenjem.
