Patogeneza helicobacter pylori infekcije. Gastritis i duodenitis izazvan helicobacter pylori infekcijom

Sažetak

Patogeneza Helicobacter pylori (Hp) infekcije zavisi od: virulencije i patogenosti bakterije, faktora sredine (higijenske i dijetetske navike stres) i osobina domaćina (starost, pol, krvna grupa). Značajnu ulogu u patogenezi Hp infekcije igraju faktori neophodni za efikasnu kolonizaciju sluznice i preživljavanje bakterije u želucu (ureaza, porini, flagele lipopolisaharid, adhezini i različiti enzimi), kao i faktori po kojima se sojevi bakterije razlikuju (ekspresija gena za vacA i cagA proteine) i koji su odgovorni za različit ishod infekcije i kliničku sliku u inficirane osobe. Gastroduodenalna inflamacija izazvana sa Helicobacter pylori podrazumeva prisustvo neutrofila, T limfocita, plazmocita i makrofaga i praćena je različitim stepenom oštećenja i degeneracije epitela. Akutni Hp gastritis prati hipohlorhidrija, a hronični povećanje kisele želudačne sekrecije izazvana hipergastrinemijom zbog smanjene ekspresije somatostatina u toku Hp infekcije. Ishod infekcije prevashodno zavisi od bakterijskog soja, distribucije gastritisa, postojanja i stepena atrofije želudačne sluzokože.
