Sažetak

Poremećaji mišljenja i kognicije spadaju u glavne karakteristike shizofrenije. Veliki broj ranijih ispitivanja je ukazao da P300 izazvani potencijal može poslužiti kao koristan dijagnostički metod za evaluaciju pacijenata za koje se pretpostavlja da imaju kognitivna oštećenja. Zato smo snimali auditivni P300 (Oddball paradigma) kod 17 shizofrenih pacijenata, koji su bili pod terapijom, i uporedili ih sa kontrolnom grupom zdravih ispitanika. Pored toga, izmerili smo kliničke karakteristike pacijenata pomoću PANSS i BPRS skala i proverili moguće korelacije između P300 parametara i ovih skala. Dominantni nalazi su bili značajno produženje srednje P3 latencije u grupi shizofrenih bolesnika i visok procenat (skoro 60%) pacijenata sa abnormalnim produženjem ove latencije. Srednje latencije ostalih vrhova su takođe bile statistički značajno produžene, ali je preklapanje pojedinačnih vrednosti između pacijenata i kontrolne grupe bilo veliko, a broj pacijenata sa abnormalnom latencijom je bio zanemarljivo mali. Nije bilo statistički značajne razlike između pacijenata i kontrolne grupe u N1-P2 i N2-P3 amplitudama. Korelacija P300 parametara sa kliničkim parametrima je otkrila tendenciju ka dužim P3 latencijama sa višim stepenom opšte psihopatologije (izmerene BPRS skorom), kao i tendenciju ka nižim N2-P3 amplitudama sa izraženijom negativnom simptomatologijom (izmerenom NSS skorom). Možemo zaključiti da P300 izazvani potencijali kod shizofrenih pacijenata predstavljaju korisno neurofiziološko sredstvo za procenu funkcionalnog integriteta struktura CNS angažovanih u kognitivnim procesima.
