Osnove parenteralne ishrane

Sažetak
Pacijenti koji ne mogu da koriste hranu preko digestivnog trakta hrane se putem parenteralne ishrane. Aktuelni stavovi Evropskog društva za parenteralnu i enteralnu ishranu (ESPEN) govore u prilog rane parenteralne ishranne već u prvih 48 časova. Intravenska upotreba hranljivih rastvora omogućava precizno doziranje i makro- i mikronutrijenata shodno potrebama pacijenta. Od suštinske je važnosti obezbediti kalorijske potrebe pacijenta balansiranom kombinacijom glukoze, lipida i aminokiselina. Pri tome je obavezno voditi računa i o zastupljenosti esencijalnih masnih kiselina i aminokiselina, kao i lipidnih i proteinskih suplemenata koji mogu imati imunomodulatorni efekat i uticati direktno na ishod lečenja. U tom smislu su danas posebno aktuelni, iako kontroverzni, riblje ulje i glutamin. Svaki rastvor za parenteralnu ishranu kritično obolelih pacijenata mora da ima i odgovarajuće količine elektrolita i mikronutrijenata (vitamina i elemenata u tragu). Za adekvatnu primenu rastvora za parenteralnu ishranu bitan je i odabir odgovarajućeg venskog puta, tehnički korektno plasiranje samog katetera i kvalitetno održavanje. Pravilno realizovana parenteralna ishrana omogućava najbolji mogući ishod za bolesnika, uz svođenje mogućnosti od nastanka komplikacija na minimum.
