Učestalost humanog papiloma virusa u orofaringealnom planocelularnom karcinomu u Srbiji

Sažetak

Uvod/Cilj Orofaringealni karcinom čini oko 3% novodijagnostikovanih karcinoma na svetu. U Srbiji 1,8% bolesnika sa malignitetom boluje od orofaringealnog karcinoma. Studije ukazuju na sve veću ulogu infekcije humanim papiloma virusom (HPV). U literaturi je pokazano da bolesnici koji su pozitivni na infekciju HPV-om imaju bolju prognozu i značajno veće ukupno preživljavanje. Cilj ovog rada je bio da ustanovi kako HPV status utiče na ukupno preživljavanje i preživljavanje bez znakova bolesti bolesnika sa orofaringealnim karcinomom u Srbiji, u zavisnosti od poznatih faktora rizika (konzumiranje alkohola i cigareta), kliničkog TNM stadijuma i načina lečenja. Metode U studiju je uključeno 87 bolesnika kojima je dijagnostikovan orofaringealni karcinom tokom perioda od jedne godine, sa petogodišnjim periodom praćenja. Terapija je uključila hirurško lečenje sa postoperativnom radioterapijom ili hemioradioterapijom, samo radioterapiju ili samo hemioradioterapiju. Ispitivani su pol, faktori rizika, klinička TNM klasifikacija, kao i uticaj HPV statusa na ukupno preživljavanje i preživljavanje bez znakova bolesti u zavisnosti od prisustva faktora rizika. Rezultati Bolesnici sa orofaringealnim karcinomom pozitivni na HPV najčešće su bili muškog pola, pušači i konzumenti alkohola. Što se tiče kliničkog TNM stadijuma, preživljavanje je bilo bolje kod HPV pozitivnih bolesnika. Bolje preživljavanje su imali HPV pozitivni bolesnici lečeni primarno hirurški u odnosu na one primarno lečene radioterapijom. Zaključak HPV status značajno utiče na preživljavanje i lokoregionalnu kontrolu kod bolesnika sa orofaringealnim karcinomom u Srbiji. Ova činjenica ukazuje na potrebu modifikovanja terapijskih strategija za ove bolesnike, u cilju daljeg poboljšavanja prognoze njihove bolesti i ishoda terapije.
