We report a case of a 72 year-old patient with a chronic left ventricular pseudoaneurysm (PAN), discovered as an incidental finding on routine echocardiographic study. The patient had a history of inferoposterolateral (IPL) myocardial infarction (AMI) 5 years ago, and subacute left ventricular free wall rupture remained undiagnosed at the time. There was also a manyyear history of rheumatoid arthritis. The patient underwent a successful operation, pseudoaneurysm was resected and the ventricular wall defect was closed with a patch. In this paper we discuss whether it was possible to recognize a subacute myocardial rupture at the time of AMI. We also discuss the link between cardiovascular disease and rheumatoid arthritis.
