Sažetak

Alfa-lipoinska kiselina (eng. Alpha-lipoic acid, ALA) je prirodni, ditiolni antioksidans koji nastaje iz oktanske kiseline. Otkrivena je 1951. godine kao molekul koji učestvuje u transferu acil grupe u Krebsovom ciklusu. ALA i njen redukovani oblik, dihidrolipoinska kiselina, danas predstavljaju važne činioce u borbi sa bolestima prouzrokovanim povećanim oksidativnim stresom. U humanom organizmu ALA se sintetiše de novo iz masnih kiselina i cisteina, ali u vrlo malim količinima, pa je neophodno da se unosi iz spoljašnjih izvora, putem hrane ili adjuvantne terapije. Završnu reakciju u sintezi ALA čini umetanje dva atoma sumpora, posredovana lipoil-sintazom, kao donorom sumpora. Endogeno sintetisana ALA je kovalentno vezana za odgovarajuće proteine, ostvarujući, na taj način, važnu ulogu kofaktora velikog broja mitohondrijalnih enzima . ALA u svojoj strukturi sadrži dve tiolne grupe, koje mogu učestvovati u reakcijama oksidacije i/ili redukcije. Karakteriše se jedinstvenim antioksidativnim sposobnostima jer i endogeno sintetisana i egzogeni oblik aktivno učestvuju u hvatanju slobodnih radikala u oksidovanoj i/ili redukovanoj formi. ALA je rastvorljiva i u vodi i u mastima, pa zato može obavljati antioksidativnu ulogu na nivou citozola, plazma membrane, serumskih i lipoproteina, učestvujući u važnim procesima modulacije signalne transdukcije uključujući insulinsku signalizaciju, PKB/Akt-zavisnu signalizaciju i redoks-osetljive transkripcione faktore. Potencijalna biohemijska i terapijska uloga ALA unete oralnim putem uslovljena je stepenom bioraspoloživosti, akumulacije u tkivima i metaboličkim procesima kojima podleže. Farmakoterapijski aspekti ALA baziraju se na učešću ovog antioksidansa u različitim antiinflamatornim signalnim putevima, pa je zato alfa-lipoinska kiselina, kao terapijski agens, prepoznata kao moćan antioksidans zauzimajući posebno mesto u terapiji kardiovaskularnih, kognitivnih, neuromuskularnih defekata i hemoprevenciji karcinoma.
