Značaj dobutaminskog stres-ehokardiografskog testa u proceni rizika kod asimptomatskog pacijenta sa tesnom aortnom stenozom i očuvanom ejekcionom frakcijom u miru - prikaz slučaja

Sažetak

Kod pacijenata sa aortnom stenozom (AS) simptomi se najčešće događaju za vreme fizičke aktivnosti. Takođe, najveći broj doppler-ehokardiografskih parametara kojima se određuje težina AS zavisi od količine protoka preko aortne valvule. Promene u dotoku krvi u levu komoru (LK) i kontraktilnosi LK mogu da utiču na težinu AS, komplijansu zidova komore, kao i na odnos između leve komore i arterijske cirkulacije. Zbog toga primena stres testa može doprineti preciznijoj dijagnostici i boljoj proceni težine AS. Ovo je prikaz slučaja asimptomatskog pacijenta sa umerenom/tesnom AS i očuvanom vrednosti ejekcione frakcije u miru kod koga je niskodozni dobutaminski stres test doprineo preciznoj proceni težine AS i pomogao da se dobije bolji uvid u optimalno vreme za zamenu aortnog zaliska.
