Uticaj manuelne aspiracije tromba na dijastolnu funkciju leve komore srca kod bolesnika s infarktom miokarda i elevacijom ST-segmenta koji su lečeni primarnom perkutanom koronarnom intervencijom

Sažetak
Uvod Podaci o efektima aspiracije tromba na dijastolnu funkciju leve komore u neselektovanoj populaciji s infarktom miokarda i elevacijom ST-segmenta (STE MI) su oskudni. Ciljrada Uporedili smo ehokardiografske odlike dijastolne funkcije i ishode kod bolesnika sa STE MI koji su lečeni manuelnom aspiracijom tromba i bez nje u akademskom centru za interventnu kardiologiju. Metode rada U studiju su uključena 433 uzastopna bolesnika kojima je rađena primarna perkutana koronarna intervencija (PPKI) u periodu 2011-2012. godine. U studi ju nisu uključeni bolesnici koji su preboleli infarkt miokarda, biliprethodno revaskularizovani, primali trombolitičku terapiju, bili u kardiogenom šoku, imali značajnu valvularnu manu, atrijalnu fibrilaciju ili ugrađen stalni vodič srčanog ritma. Ehokardiogram je urađen tokom 48 časova od PPKI. Stanje bolesnika je klinički praćeno ambulantno ili se o njemu obaveštavalo telefonskim putem. Rezultati Ispitanici lečeni aspiracijom tromba (TA+, 216 bo lesnika) imali su slične osnovne karakteristike kao ispitanici lečeni bez aspiracije tromba (TA-, 217 bolesnika) i slično ukupno ishemijsko vreme (319±276 prema 333±372 minuta; p=0,665), volumene isistolnu funkciju leve komo re, ali je TA+ grupa imala veće maksimalne vrednosti troponina I (39,5±30,5 pre ma 27,6±26,9 ng/ml; p<0,001) i znatno veću učestalost E/e'>15 (23,1% prema 15,2%; p=0,050). Tokom perioda nadgledanja, koje je trajalo 14±5 meseci, učestalost velikih neželjenih srčanih, odnosno moždanih događaja bila je slična u obe grupe ispitanika (p=0,867). Zaključak Aspiracija tromba je povezana s većom učestalošću teške dijastolne disfunkcije leve komore kod bolesnika sa STEMI koji su lečeni primenom PPKI, ali nema uticaja na učestalost velikih neželjenih kardiocerebrovaskularnih događaja.
