Promene u retrobulbarnoj arterijskoj cirkulaciji kod muškaraca i žena s primarnim glaukomom otvorenog ugla nakon sniženja povišenog intraokularnog pritiska

Sažetak

Uvod. Pretpostavlja se da je poremećaj perfuzije u glavi očnog živca patogeni faktor kod glaukoma. Cilj rada. Cilj rada je bio da se procene promene u hemodinamskim parametrima retrobulbarne arterijske cirkulacije nakon snižavanja prethodno povišenog intraokularnog pritiska (IOP) kod muškaraca i žena s primarnim glaukomom otvorenog ugla (engl. primary open-angle glaucoma - POAG). Metode rada. Istraživanjem je obuhvaćeno 60 ispitanika (33 muškarca i 27 žena) starijih od 50 godina sa dijagnostikovanim i lečenim POAG. Na 77 očiju 39 bolesnika zabeležen je povišen IOP (>25 mm Hg). Ispitivanje je obavljeno od decembra 2009. do decembra 2010. godine na Klinici za očne bolesti (kompletan oftalmološki pregled) i na Klinici za neurologiju Kliničkog centra Srbije u Beogradu. Kolor doplerom su ispitani hemodinamski parametri tri retrobulbarna arterijska krvna suda: oftalmičke arterije, centralne retinalne arterije i kratkih cilijarnih arterija. Rezultati. Kod žena hemodinamski arterijski parametar vršne sistolne brzine protoka krvi porastao je u centralnoj retinalnoj arteriji, a smanjio se u oftalmičkoj i kratkim zadnjim cilijarnim arterijama. Završna dijastolna brzina protoka krvi je porasla u sva tri retrobulbarna krvna suda. Purseloov (Pourcelot) indeks rezistencije se povećao, dok se indeks pulzatilnosti snizio u sva tri krvna suda. Kod muškaraca su se vršna sistolna i završna dijastolna brzina protoka krvi i indeks pulzatilnosti snizili u sva tri krvna suda. Indeks rezistencije se povećao u oftalmičkoj arteriji, a smanjio u centralnoj retinalnoj i kratkim zadnjim cilijarnim arterijama. Statistička značajnost je zabeležena u indeksu pulzatilnosti u oftalmičkoj arteriji žena i u završnoj dijastolnoj brzini protoka krvi u oftalmičkoj arteriji muškaraca. Zaključak. Utvrđena je razlika u retrobulbarnoj arterijskoj cirkulaciji između muškaraca i žena sa POAG nakon sniženja IOP. Uloga vaskularnih faktora u ishrani neuroretinalnog oboda glave očnog živca je veoma značajna.
