Invazivni sojevi Streptococcus pneumoniae u Srbiji - osetljivost na antibiotike i serotipovi

Sažetak

Uvod. Streptococcus pneumoniae je jedan od vodećih uzročnika bakterijskog meningitisa i sepse. Invazivna pneumokokna bolest je značajan medicinski problem zbog velikog povećanja rezistencije pneumokoka na antibiotike. Cilj rada. Cilj istraživanja je bio da se ispita osetljivost invazivnih izolata pneumokoka na antibiotike i odrede serotipovi rezistentnih sojeva. Metode rada. Ispitana je osetljivost 58 invazivnih izolata S. pneumoniae na 19 antibiotika. Serotipizacija je urađena reakcijom aglutinacije. Sojevi su prikupljani u periodu 2009-2011. godine u Nacionalnoj referentnoj laboratoriji za streptokok i pneumokok. Rezultati. Smanjena osetljivost na penicilin je utvrđena kod 34% izolata pneumokoka, a na eritromicin kod 36%. Pedijatrijski izolati su pokazivali znatno veću učestalost rezistencije u odnosu na sojeve izolovane od odraslih bolesnika. Na penicilin je bilo rezistentno 65% izolata od dece i 22% od odraslih, dok je 47% pedijatrijskih izolata i 32% odraslih izolata bilo rezistentno na eritromicin. Istovremena rezistencija na oba antibiotika uočena je kod 21% sojeva, a multirezistencija kod trećine izolata. Svi sojevi su bili osetljivi na vankomicin, linezolid, fluorohinolone, telitromicin i rifampicin. Najučestaliji rezistentni serotipovi bili su 19F i 14. Zaključak. Rezultati su pokazali da je rezistencija invazivnih sojeva pneumokoka na penicilin i makrolide u Srbiji visoka. Stoga bi bilo neophodno uvesti aktivan nadzor nad invazivnim pneumokoknim bolestima, tipizirati takve izolate i neprestano pratiti njihovu rezistenciju na antibiotike.
