Značaj mikrobiološkog ispitivanja vaginalnog sekreta u reproduktivnom periodu

Sažetak

Vaginalne infekcije u reproduktivnom periodu žena su česte i, mada ne ugrožavaju život, značajno mogu da utiču na njihovo normalno funkcionisanje.Takođe, mogu da utiču i na fertilnost žena, kao i na tok i ishod trudnoće zbog mogućnosti infekcije novorođenčeta tokom prolaska kroz porođajni kanal inficirane majke. Bakterijska vaginoza ce statistički značajno češće nalazi kod pacijentkinja sa pre kanceroznim promenama na grliću materice ili sa karcinomom cerviksa, u poređenju sa ženama sa nepromenjenim grlićem, a može da uzrokuje i znatno češću pojavu postoperacionog pelvičnog celulitisa nakon histerektomije. S druge strane, prisustvo S. agalactiae u vaginalnom sekretu može biti značajan uzročnik veoma ozbiljnih i po život opasnih infekcija novorođenčeta, kao što su meningitis, pneumonija i sepsa. Za protozou T. vaginalis dokazano je da može da utiče na smanjenje plodnosti, a u toku trudnoće može da ošteti ovojnice ploda, dovede do njihovog prevremenog prskanja i uzrokuje prevremeni porođaj, a postoji i povećan rizik od nastanka karcinoma grlića materice. U reproduktivnom periodu žena, a naročito ukoliko postoje određeni faktori rizika kao što su hormonska terapija, dijabetes melitus tip 1 i upotreba antibiotika širokog spektra, česte su gljivične infekcije uzrokovane gljivama roda Candida. One, osim što dovode do nelagodnosti kao što su pojačan svrab i sekret, često predstavljaju dijagnostički i terapijski problem. Zbog toga ce preporučuje redovna mikrobiološka kontrola žena u reproduktivnom periodu na prisustvo bakterija, uzročnika bakterijske vaginoze, gljiva i trihomonasa. Tek ukoliko su ovi nalazi negativni, sprovode ce dalja mikrobiološka ispitivanja, koja su komplikovanija, duže traju i znatno su skuplja, kao što su ispitivanja na hlamidije, mikoplazme i neke viruse koji mogu uzrokovati vaginalne infekcije kod žena u reproduktivnom periodu.
