Jednogodišnje iskustvo primene PCI sa ugradnjom TAXUS stentova u lečenju višesudovne koronarne bolesti srca u odnosu na aortokoronarnu bajpas operaciju

Sažetak

Uvod. Lečenje višesudovne koronarne bolesti primenom perkutane koronarne intervencije (PCI) pokazalo je zadovoljavajući kratkoročni, ali inferiorniji dugoročni rezultat u odnosu na aortokoronarnu bajpas operaciju (CABG). Određene grupe pacijenata mogu imati podjednako dobar dugoročan rezultat primene PCI u lečenju višesudovne bolesti kao i pacijenti sa CABG. Prilikom izbora metode revaskularizacije osim angiografskog nalaza potrebna je evaluacija kliničkog stanja pacijenta, ciljeva lečenja, dosadašnjih saznanja i iskustava kako bi se donela najpovoljnija odluka za pacijenta. Cilj studije je da prikaže ishod jednogodišnjeg praćenja pacijenata sa višesudovnom koronarnom bolešću lečenih PCI sa ugradnjom TAXUS stentova u odnosu na pacijente koji su lečeni primenom CABG. Metode. Radom je obuhvaćeno 114 pacijenata u PCI grupi i 93 pacijenta kod kojih je urađena CABG tokom 2004. i 2005. godine. Ishod lečenja praćen je brojem ukupnih kardijalnih i cerebrovaskularnih događaja nakon intervencija (MACCE). Rezultati. Posle godinu dana praćenja MACCE u PCI grupi iznosio je 17,5%, od čega je samo učestalost reintervencija bila 11,5%. U CABG grupi MACCE je iznosio 14%, od čega su reintervencije činile 4,3%. Nije bilo statistički značajne razlike između učestalosti MACCE-a (p=0,486) niti između učestalosti reintervencija između grupa (p=0,064). Zaključak. Primena PCI sa ugradnjom TAXUS stentova za lečenje višesudovne koronarne bolesti podjednako je efikasna kao i CABG godinu dana posle intervencije.
