Antibakterijska rezistencija urinarnih bakterija kod novorođenčadi i male dece obolele od akutnog pijelonefritisa

Sažetak

Uvod. Infekcija urinarnog trakta je česta u detinjstvu. U zavisnosti od mesta infekcije, jačine njene kliničke slike i mogućih akutnih i dugoročnih komplikacija, ona se opisuje kao akutni cistitis ili akutni pijelonefritis. Cilj rada. Cilj rada bio je da ispita rezistencija urinarnih bakterija tokom pet godina kod novorođenčadi i male dece obolele od akutnog pijelonefritisa. Metode rada. Retrospektivno je analizirana rezistencija uropatogenih bakterija na antibakterijske lekove (ampicilin, kombinaciju sulfometoksazola i trimetoprima, cefaleksina, ceftriaksona, cefotaksima, ceftazidima, gentamicina, amikacina, ciprofloksacina, imipenema i nalidiksične kiseline) kod novorođenčadi i male dece tokom ranog (2005-2007) i kasnijeg (2008-2009) studijskog perioda. Za određivanje antibakterijske senzitivnosti u urinu korišćena je standardna disk-difuziona metoda. Rezultati. Ukupno 117 novorođenčadi i 294 mala deteta uzrasta 9,3±0,7 meseci lečeno je tokom ranog (136 bolesnika) i kasnog (275 bolesnika) studijskog perioda. Escherichia coli je bila najčešći uzročnik infekcije u oba perioda (85,5%). U odnosu na decu stariju od mesec dana, kod novorođenčadi je utvrđen veći stepen antibakterijske rezistencije na cefalosporine druge i treće generacije, aminoglikozide i nalidiksičnu kiselinu za vreme ranog studijskog perioda, a na ceftazidim, aminoglikozide i nalidiksičnu kiselinu tokom kasnijeg perioda ispitivanja. Rezistencija na nekoliko lekova bila je takođe češća kod novorođenčadi u ranom studijskom periodu. Zaključak. Kod novorođenčadi je veći stepen antibakterijske rezistencije nego kod dece starije od mesec dana. Zabrinjava, međutim, povećanje učestalosti antibakterijske rezistencije kod dece s prvim akutnim pijelonefritisom.
