Klinički značaj određivanja antitela na ciklične citrulinisane peptide u sistemskoj sklerozi

Sažetak

Uvod. Antitela na ciklični citrulinisani peptid (anti-CCP) visoko su specifična za reumatoidni artritis i važan prognostički marker za razvoj erozivnog artritisa. Neke studije su dokazale njihovu malu učestalost kod osoba obolelih od sistemske skleroze (SS) s artritisom. Cilj rada. Cilj rada bio je da se utvrdi učestalost anti- CCP antitela kod bolesnika sa SS i njihova povezanost s kliničkim manifestacijama i radiografskim znacima, posebno na zglobovima šaka. Metode rada. Ispitivanje je obuhvatilo 82 bolesnika sa SS (59 sa limitiranom SS i 23 sa difuznim oblikom SS), prosečne starosti od 54,4 godine, i 28 zdravih ispitanika iste starosti, koji su činili kontrolnu grupu. Svim ispitanicima određeni su anti- CCP antitela i reumatoidni faktor (RF) i načinjen radiografski snimak šaka s ručnim zglobovima. Rezultati. Anti- CCP antitela su utvrđena kod 11 bolesnika (13,4%), dok kod ispitanika kontrolne grupe nisu nađena. Pozitivan RF utvrđen je kod 15,9% bolesnika. Artritis je dijagnostikovan kod 17,1% bolesnika, a marginalne erozije (radiografski) kod istog broja bolesnika. Utvrđena je statistički značajna povezanost pozitivnih anti- CCP antitela s artritisom (p<0,0001) i marginalnim erozijama (p=0,0002). Zaključak. Anti- CCP antitela su povezana s kliničkim znacima artritisa i radiografskim oštećenjem zglobova šaka. Koristan su marker u prepoznavanju osoba obolelih od SS s artritisom i anatomskim oštećenjem kosti, radi primene odgovarajućeg lečenja, čiji je cilj prevencija oštećenja zglobova i lošeg kvaliteta života bolesnika.
