Imunološka rekonstitucija nakon transplantacije matičnih ćelija hematopoeze - parametri monitoringa i savremene metode njene evaluacije

Sažetak
Stanje izrazite imunoinkompetencije, uprkos funkcionisanju transplantata matičnih ćelija hematopoeze, karakteristika je ranog posttransplantacionog perioda. Imunološke funkcije recipijenta su oslabljene tokom nekoliko meseci nakon transplantacije, što je u zavisnosti od starosti bolesnika, osnovne bolesti, stepena nepodudarnosti u HLA i u minor histokompatibilnim antigenima, porekla hematopoeznih ćelija, pripreme grafta (netretirani graft,T-ćelijski depletiran, krv iz pupčanika)i komplikacija nastalih u posttransplantacionom periodu. Najvažniji imunološki testovi upotrebljavani za praćenje i procenu imunološkog oporavka su: fenotipiziranje limfocitnih subpopulacija T, B limfocita i NK ćelija, ispitivanje njihove funkcionalnosti, produkcije imunoglobulina, ispitivanja komplementa efektornih mehanizama limfoidnih ćelija kao i produkcije citokina. U novije vreme u upotrebi su savremene tehnike: ELISPOT tehnika IMMUNOSCOPE/SPECTRATYPING metoda i TREC tehnologija. Zahvaljujući upotrebi kako standardnih tako i savremenih tehnika, omogućena je procena dinamike i praćenje imunološkog oporavka u recipijenta, a naročito praćenje rekonstitucije varijabilnosti produkata gena T-ćelijskih receptora, uloge timusa i antigen specifičnog T-ćelijskog odgovora nakon transplantacije matičnih ćelija hematopoeze.
