Kratkoročni ishod prevremeno rođene dece u zavisnosti od toga da li su rođena iz jednostrukih, blizanačkih ili trigemelarnih trudnoća - podaci iz tercijarne zdravstvene ustanove u Srbiji

Sažetak
Uvod Posle uvođenja nacionalnog programa lečenja neplodnosti zapažen je povećan broj višeplodnih trudnoća. Nalazi iz literature su sporni u pogledu postojanja povećanog rizika morbiditeta i mortaliteta kod prevremeno rođene novorođenčadi iz višeplodnih trudnoća. Metode U studiju su uključena prevremeno rođena novorođenčad iz jednoplodnih, blizanačkih i trigemelarnih trudnoća, rođena tokom dvogodišnjeg perioda trajanja studije. Najpre su pronađeni podaci o svim prevremeno rođenim blizancima. Za svaki par blizanaca odabrano je dvoje prevremeno rođene novorođenčadi iz jednoplodnih trudnoća, rođenih posle istog trajanja gestacije. Set ispitanika je upotpunjen uključivanjem sve prevremeno rođene dece iz trigemelarnih trudnoća. Poređeni su kratkoročni ishodi ove tri grupe - singli, blizanci i trojke. Rezultati U studiju je uključeno ukupno 210 prevremeno rođene dece, od kojih 84 iz jednoplodnih trudnoća, 84 blizanca i 42 trojke. Statistička analiza je pokazala značajnu razliku među grupama u pogledu načina začeća (p > 0,0001), načina porođaja (p < 0,001) i telesne mase na rođenju (p = 0,005). Kratkoročni mortalitet i morbiditet (smrt u novorođenačkom uzrastu, potreba za intubacijom na rođenju, respiratornom potporom i terapijom surfaktantom, intrakranijalno krvarenje) bili su znatno češći kod trojki u poređenju sa singlima i blizancima. Zaključak Prevremeno rođene trojke imaju veći rizik za nepovoljni kratkoročni ishod u poređenju sa singlima i blizancima približne gestacije u našem uzorku ispitanika.
