Somatostatinom Vaterove papile kod bolesnice s von Reklinghausenovom bolesti

Sažetak
Uvod. Somatostatinomi digestivnog trakta luče hormon somatostatin koji može izazvati klinički sindrom poznat kao "inhibitorni sindrom" koji uključuje dijabetes melitus, holelitijazu i steatoreičnu dijareju. Somatostatin luče i D ćelije Langerhansovih ostrvaca, ali i endokrine ćelije u želucu, tankom crevu i pljuvačnim žlezdama kao i tzv. parafolikulne ćelije tiroidne žlezde. Somatostatinomi digestivnog trakta javljaju se u pankreasu i duodenumu. Osobe sa von Reklinghausenovom bolešću su naročito sklone somatostatinomima duodenuma. Prikaz bolesnika. Prikazana je bolesnica stara 51 godinu s von Reklinghausenovom bolešću kod koje je, tokom ispitivanja uzroka opstrukcione žutice, utvrđen tumor Vaterove papile. Učinjena je cefalična duodenopankreatektomija po Whippleu i ekscizija nekoliko neurofibroma sa zida početnog dela jejunuma i sa kože. Histološkim i imunohistohemijskim ispitivanjem tumora utvrđeno je da se radi o tipu B glandulnog karcinoidnog tumora s jakom antisomatostatinskom i pratećom lakom antigastrinskom imunoreaktivnošću. Više od četiri godine posle operacije bolesnica je bez simptoma. Zaključak. Kod bolesnika s von Reklinghausenovom bolešću treba tragati za karcinoidnim tumorom duodenuma i papile, pogotovo u slučaju pojave znakova holestaze.
