Učestalost i antimikrobna osetljivost urinarnih izolata Escherichia coli koji proizvode beta-laktamaze proširenog spektra dejstva u vanbolničkoj populaciji

Sažetak
Uvod. Proizvodnja beta-laktamaza je najznačajniji mehanizam rezistencije Gram-negativnih bakterija na beta-laktamske antibiotike. Na području banjalučke regije nisu rađena opsežnija istraživanja koja bi dala potpunu sliku o učestalosti i rezistenciji izolata Escherichia coli (E. coli) koji proizvode beta-laktamaze proširenog spektra dejstva (ESBL). Cilj rada. Cilj rada je bio da se utvrde prisustvo izolata E. coli koji proizvode ESBL kao uzročnika urinarnih infekcija u vanbolničkoj populaciji, njihova raspodela prema polu i starosti bolesnika i njihova osetljivost na antimikrobne lekove. Metode rada. Iz uzoraka mokraće vanbolničkih ispitanika zasejanih na hromogene ploče CPS-ID 3 ispitivani su izolati E. coli izolovane u čistoj kulturi i u znatnom broju (≥105 bakterija po mililitru urina). Osetljivost na antimikrobne lekove rađena je na aparatu VITEK TWO Compact pomoću kartice AST-GN27 za ispitivanje osetljivosti Gram-negativnih bakterija i otkrivanje izolata koji proizvode ESBL. Rezultati. Od ukupno 2.195 izolata, 177 (8,1%) je proizvodilo ESBL, pri čemu su kod žena izolovana 92 izolata (5% od ukupnog broja izolata E. coli kod žena), a kod muškaraca 85 izolata (23% od ukupnog broja izolata E. coli kod muškaraca). Visok procenat ovih izolata nađen je kod dece uzrasta do godinu dana (36,7%) i kod osoba starijih od 60 godina (28,8%). Svi izolati koji proizvode ESBL bili su osetljivi na imipenem i rezistentni na ampicilin, piperacilin, cefazolin, cefotaksim, ceftazidim i cefepim. Uočena je značajna rezistencija na amikacin (79,1%), gentamicin (76,8%), amoksicilin sa klavulanskom kiselinom (54,80%) i trimetoprim-sulfametoksazol (45,76%). Rezistencija na nitrofurantoin bila je 13,5%. Zaključak. U radu je dokazano postojanje urinarnih izolata E. coli koji proizvode ESBL u vanbolničkoj populaciji, a utvrđena je i njihova izražena osetljivost na imipenem i nitrofurantoin.
