Veza između graničnog poremećaja ličnosti i posttraumatskog stresnog poremećaja

Sažetak

Niz istraživanja podržava nalaz značajne povezanosti između granične psihopatologije i ponavljanih traumatskih iskustava. U literaturi se pominje i podatak da više od polovine svih pacijenata sa graničnim poremećajem ličnosti imaju u anamnezi podatak o teškom psihičkom ili seksualnom zlostavljanju pre svoje šeste godine. Intenzivna trauma, posebno ukoliko je potisnuta, može usloviti pojavu hroničnog posttraumatskog stresnog poremećaja koji utiče na celu ličnost, tako da mnogi pacijenti sa dijagnozom posttraumatskog stresnog poremećaja pokazuju i simptome karakterne patologije (vrlo često i visok nivo graničnosti). U radu se iznosi pregled literature vezane za značaj i ulogu traume u razvoju graničnog poremećaja ličnosti koji i na fenomenološkom planu ispoljavaju značajnu sličnost. Mada linearna kauzalnost u razmatranju veze između ovih poremećaja i traume predstavlja simplifikaciju kompleksne interakcije temperamenta, specifičnosti same traume, kvaliteta spoljnjeg okruženja, specifičnosti obrazaca relacija sa drugima pre traume i sl., bolje razumevanje međuigre razvojnih procesa i traumatskog stresa omogućava bolje razumevanje etiopatogeneze poremećaja ličnosti, a dualna dijagnoza pacijenata sa posttraumatskim stresnim poremećajem omogućava izbor specifičnije i efikasnije forme tretmana.
