Lipidni status i indeks telesne mase kao prognostički faktori u pacijenata sa amiotrofičnom lateralnom sklerozom

Sažetak

Uvod: Amiotrofična lateralna skleroza (ALS) najozbiljniji je oblik degenerativne bolesti motornih neurona kod odraslih, čiji nezaustavljivi tok vodi ka smrti za dve do pet godina, najčešće zbog respiratorne insuficijencije. Osim starosti i mesta početka bolesti, nijedan drugi činilac nije dokazan kao pouzdan pokazatelj toka i ishoda bolesti, odnosno vremena preživljavanja. Cilj: Procena uticaja serumskih vrednosti holesterola, triglicerida i indeksa telesne mase na početku praćenja bolesti, na preživljavanje obolelih od ALS. Materijal i metode: Ispitivanje je obuhvatilo 234 pacijenta Klinike za neurologiju Kliničkog centra Srbije sa ALS koji su dijagnostikovani i praćeni u periodu od 8 godina (2004-2011). Kompletan lipidni status (nivo holesterola i triglicerida) i indeks telesne mase imalo je 59 pacijenata kod kojih je ispitivan uticaj ovih parametara na preživljavanje. Kaplan-Majerovom metodom, odnosno Mantel-Koksovim testom je procenjeno trajanje bolesti, odnosno srednje preživljavanje. Rezultati: U ovoj studiji 19 (32,2%) pacijenata imalo je fiziološke vrednosti lipida a 40 (67,8%) hiperlipidemiju. Srednje preživljavanje pacijenata sa Normalnim lipidima od pojave simptoma iznosilo je 3,21 ± 1,44 godine, a sa hiperlipidemijom 3,3 ± 1,59 godine (p = 0,71). U odnosu na pol, životno doba i formu bolesti na početku nije uočena značajna razlika u preživljavanju pacijenata. Takođe, nije pokazano da indeks telesne mase značajno utiče na preživljavanje. Zaključak: Rezultati dobijeni ovom studijom pokazuju da se hiperlipidemija i veće vrednosti indeksa telesne mase ne mogu dovesti u vezu sa dužim preživljavanjem pacijenata obolelih od ALS, te da nisu pouzdan prognostički faktor.
