Significance of 99mTc-DTPA and 99mTc-DMSA application for assessment of renal function in patients with primary glomerulonephritis

Abstract

99mTc-DTPA and 99mTc-DMSA were employed to assess renal function in 36 patients with primary glomerulonephritis and to estimate relationship between glomerular and tubular function at different levels of glomerular filtration rate. Findings obtained were compared with those of 23 healthy persons. 99mTc-DTPA clearance and 99mTc-DMSA renal uptake were significantly lowered in glomerulonephritic patients related to controls. In patients with 99mTc-DTPA clearance values above 107 ml/min ( > control x - 2 SD), as well as in patients with clearance values 80-107 ml/min, 56-79 ml/min and < 56, mean 99mTc-DMSA uptake values were more pronounced reduced than mean 99mTc-DTPA clearance values. Mean values of 99mTc-DTPA clearance in these groups related to control (100%) were 90,2%, 73,2%, 57,4% and 30,3%, and corresponding mean values of 4 h 99mTc-DMSA uptake were 74%, 56,0%, 52,4% and 23,6%. Good correlation was proved between 99mTc-DTPA clearance and 4 h 99mTc-DMSA uptake in patients with clearance over 107 ml/min, 80-107 ml/min and bellow 56 ml/min. Results obtained showed decrement of glomerular as well as tubular function in early stage of disease. Tubular function is more marked lowered than glomerular function at all levels of glomerular filtration rate. High correlation between 99mTc-DTPA clearance and 99mTc-DMSA tubular uptake at various ranges of glomerular filtration rate point out parallelism of glomerular and tubular dysfunction in primary glomerulonephritis.
