Endokrina funkcija masnog tkiva i njen klinički značaj

Sažetak
Masno tkivo sekretuje brojne bioaktivne peptide-adipokine i aktivno je uključeno u regulaciju ćelijskih funkcija, kroz kompleksnu mrežu endokrinih, parakrinih i autokrinih signala. Ovi signali utiču na odgovor mnogih tkiva i organa, uključujući hipotalamus, pankreas, jetru, skeletne mišiće, bubrege, endotel i imuni sistem. Povećana produkcija većine adipokina utiče na brojne funkcije, poput apetita, energetskog balansa, imuniteta, osetljivosti na insulin, angiogenezu, krvni pritisak, metabolizam masti i homeostazu glukoze. Leptin, do sada najbolje proučeni hormon masnog tkiva je kritički medijator energetskog balansa. Premda su mnogi efekti delovanja leptina posledica njegovog neposrednog delovanja na hipotalamus, funkcionalni leptinski receptor (duga forma) utvrđen je u mnogim tkivima van CNS-a, uključujući i imune ćelije. Izgleda da su gojazne osobe rezistentne na hipotalamičke efekte leptina (možda zbog problema u krvno-moždanoj barijeri), te nema smanjenja apetita, niti porasta potrošnje energije, a gojaznost se održava. Adipokini poput adiponektina i leptina, bar u fiziološkim koncentracijama imaju dejstva slična insulinu, zato što stimulišu beta oksidaciju masnih kiselina u skeletnim mišićima. Uloga rezistina je manje istražena. Ukazano je na njegov značaj na insulinsku rezistenciju kod pacova, ali za sada ne i kod čoveka. Adiponektin i rezistin imaju značaj za metaboličku homeostazu. NJihova koncentracija u plazmi je inverzno (adiponektin) ili direktno (rezistin) povezana sa gojaznošću (i kod dijabetesa tip 2), a efekat na insulinsku rezistenciju im je takođe oponentan. Adipociti sekretuju adipsin, faktor B, i faktor C3. Sve u svemu, bolje razumevanje patofiziologije i molekularnog delovanja adipokina, u narednim godinama može voditi efikasnijim terapijskim strategijama.
