Srčani zastoj na sportskim terenima - Eureca one 2014-2015

Sažetak
Cilj: Cilj rada je da se analizira učestalost iznenadnog srčanog zastoja na sportskim terenima u Srbiji. Metod: Obrađeni su rezultati dobijeni učešćem u prospektivnoj studiji EuReCa 2014. i EuReCa 2015. Prospektivni opservacioni trijal Evropskog Resuscitacionog Saveta (ERC) je registrovan u bazi trijala pod brojem NCT02236819 i odobren od zdravstvenih autoriteta u SAD. Rezultati: U posmatranom dvogodišnjem periodu, iznenadni srčani zastoj na sportskim terenima je zabeležen kod 5 pacijenata. Kod svih pacijenata je intervenisala SHMP uz potvrdu iznenadnog srčanog zastoja. Svi pacijenti su osobe muškog pola. Najmlađa osoba je imala 22 a najstarija 50 godina. Studija ne obuhvata podatke koji ukazuju da li je ISZ nastupio kod sportista na terenu ili u publici. Svi ISZ su bili osvedočeni od strane svedoka-laika. Ni u jednom slučaju KPR nije započet od strane svedoka. Kod 4 pacijenta je zabeležen nešokabilan inicijalni ritam. AED aparat nije korišten ni kod jednog pacijenta. Kod jednog pacijenta je isporučen DC šok od strane hitne medicinske pomoći. ROSC je postignut kod 4 pacijenta i oni su transportovani u bolnicu. Nema podataka o preživljavanju nakon mesec dana. Zaključak: ISZ nije čest uzrok zastoja na sportskim terenima u našoj zemlji. Međutim, masovna obuka i podizanje svesti građana o merama KPR postupaka kao i potreba za implementacijom AED aparata koji bi bili dostupni i na sportskim terenima, omogućilo bi pravovremeno uključivanje i odgovarajuće reagovanja laika.
Abstract
Objective: In this study we tried to establish the incidence of OHCA on sports grounds in Serbia. Method: We analyzed the results gathered through the EuReCa 2014 and 2015 study. It is a prospective observational trial of the European Resuscitation Council registered in the trial database - NCT02236819 and approved by health authorities in USA. Results: During the observed period of 15 month OHCA occurred 7 times on a sports ground in Serbia. In every single case an EMS team intervened. All of the patients were male. The youngest one was 22 years old, and the oldest was 50. The study doesn't consider whether the incident happened to an athlete on the court, or among the spectators. All of the incidents were witnessed, but no one started CPR. Four patients initially had non shockable rhythm. AED was not used in any of the cases. In one case a DC shock was delivered by EMS team. ROSC was achieved in 5 patients, and they were transferred to hospital. There is no record of one month surviving. Conclusion: OHCA is not common during sport events in our country. However, training and raising awareness of people about CPR procedures, as well as the implementation of the AEDs available on sports grounds, could provide bystander's involvement at a right time and an adequate response.
