Aneurizma unutrašnje karotidne arterije po tipu "krofne" tretirana pomoću stenta DERIVO za preusmeravanje protoka

Sažetak
Uvod Intrakranijalne aneurizme sa radiološkim znakom krofne su medicinski prioritet i dosad su opisane u malom broju slučajeva. Ovaj radiološki znak se javlja u aneurizmama gde postoje parcijalna tromboza unutar aneurizmatske vreće i cirkularni laminarni tok krvi između zida aneurizme i centralnog tromba. Posledično, na različitim angiografskim vizuelizacionim metodama postoji centralni defekt u punjenju aneurizme kontrastnim sredstvom. Prikaz bolesnika Predstavljamo bolesnicu staru 35 godina, primljenu na ispitivanje zbog čestih glavobolja, vizuelnih smetnji na levom oku i gubitka vida na desnom oku. Digitalna suptrakciona angiografija prikazala je aneurizmu na desnoj unutrašnjoj karotidnoj arteriji, koja je merila 25,6 × 25 mm, dok je promer vrata bio 11 mm i obuhvatao je deo segmenata C6 i C7. Odlučeno je da bi pozicioniranje stenta sa efektom preusmeravanja protoka preko vrata aneurizme bilo najbolje u ovom slučaju. Nakon procedure bolesnica je otpuštena nepromenjenog opšteg stanja. Sedam meseci nakon intervencije bolesnica je primljena na prvu kontrolnu digitalnu suptrakcionu angiografiju. Levi segment A1 se vratio u normalnu poziciju, sugerišući smežuravanje aneurizmatske vreće. Takođe, bolesnica je navela poboljšanje vida na oba oka. Zaključak Prikazujemo bolesnicu sa aneurizmom oblika krofne na desnoj unutrašnjoj karotidnoj arteriji, uspešno tretiranoj stentom za preusmeravanje protoka.
