Razlozi neupotrebljivosti lipemične krvne plazme u transfuziološkom lečenju

Sažetak
Uvod/Cilj Povišeno prisustvo lipidnih čestica u krvi je jedan od najčešćih razloga neupotrebljivosti transfuzioloških jedinica. Faktori rizika za lipemičnu plazmu kod davalaca nisu potpuno poznati. Cilj rada je bio utvrđivanje faktora koji utiču na nastanak lipemične plazme, čime bi se sprečili, posledično, dalji troškovi skladištenja i uklanjanja neupotrebljivih transfuzioloških jedinica. Metode Studija slučaja i kontrola sprovedena je tokom 2017. godine i uključena su 1552 ispitanika. Kontrolna grupa je obuhvatala 1502 ispitanika čija krv nije bila lipemična, dok je u ispitivanu grupu odabrano 50 bolesnika sa lipemičnom krvlju. Prisustvo lipemične krvi utvrđivano je inspekcijskim pregledom, dok su podaci prikupljeni laboratorijskim testovima i anketnim upitnikom. Rezultati Naši nalazi pokazuju da višestruki davaoci krvi, čija je krv lipemična, pripadaju sledećim kategorijama: značajno stariji (p < 0,0005), imaju viši sistolni i dijastolni pritisak (p < 0,0005), visok nivo triglicerida (p < 0,0005) i niži nivo hemoglobina (p < 0,0005). Takođe, prisustvo lipemične plazme povezano je sa ženskim polom (p = 0,002), krvnom grupom (p = 0,016), bolestima srca (p < 0,0005), pušenjem (p < 0,005), dijabetesom (p = 0,001), kao i sa unosom masne hrane pre davanja krvi (p < 0,005) i terapije venepunkturom (p < 0,0005). Sistolni pritisak je pouzdan prediktor lipemične krvi (AUROC = 0,901, p < 0,0005). Zaključak Naša studija je pružila racionalno objašnjenje i identifikovala je neke od faktora rizika koji mogu pomoći u identifikaciji mogućih davalaca lipemične krvi.
