Faktori rizika za nastanak bolničkih infekcija u jedinicama intenzivne nege

Sažetak
Uvod. U jedinicama intenzivne nege bolesnici su izloženi visokom riziku za nastanak bolničkih infekcija, zbog težine osnovne bolesti ali, takođe, i zbog rizika povezanog sa invazivnim medicinskim procedurama kojima se podvrgavaju. Cilj rada bio je da se sagleda učestalost bolničkih infekcija u jedinicama intenzivne nege Opšte bolnice Užice i utvrde faktori rizika za njihov nastanak. Metode. Prospektivnim nadzorom nad bolničkim infekcijama, sprovedenim u periodu od 27.6. do 31.12.2001. godine obuhvaćeno je 914 bolesnika koji su proveli najmanje 24 sata u jedinicama intenzivne nege (ukupno 2 615 dana). Nadzor je vršen svakodnevno od trenutka prijema u jedinicu intenzivne nege do 48 časova po otpustu iz ove jedinice. Za procenu faktora rizika za nastanak bolničkih infekcija sproveli smo anamnestičku studiju. Podaci koji se odnose na spoljašnje i unutrašnje faktore rizika za nastanak bolničkih infekcija redovno su prikupljani od svih bolesnika. Rezultati. U posmatranom periodu incidencija bolničkih infekcija iznosila je 16,7% ili 58,5 na 1 000 bolesnik-dana. Najučestalije su bile infekcije operativnog mesta (32,6%), infekcije mokraćnog sistema (23,5%) i infekcije krvi (7,1%). Kao nezavisni faktori rizika za njihov nastanak identifikovani su: hirurška intervencija (OR = 5,74; CI = 2,01-16,41), endotrahejni tubus (OR = 3,40; CI = 1,07-10,89), cistoskopija (OR = 2,35; CI = 1,38-4,02), gojaznost (OR = 1,98; CI = 1,27-3,11) i dužina primene infuzije (OR = 1,34; CI = 1,23-1,46). Zaključak. Najznačajniji faktor rizika za nastanak bolničkih infekcija u jedinicama intenzivne nege Opšte bolnice u Užicu bili su hirurška intervencija i primena endotrahejnog tubusa.
