Lateralni ekstrakavitarni pristup kičmenom stubu

Sažetak
Uvod. U radu je prikazan lateralni ekstrakavitarni pristup lumbalnom regionu kičmenog stuba u položaju tročetvrtinske pronacije. Zbog nedostataka posteriornih pristupa kod bolesnika s ventralno lociranim patološkim supstratom, dolazi do razvoja anteriornih pristupa, a potom i lateralnog ekstrakavitarnog pristupa lumbalnoj i torakalnoj regiji kičmenog stuba. Prikaz bolesnika. Operisan je bolesnik s tumorom (chordom) pršljenskog tela L3 s ventralnom kompresijom elemenata kaude ekvine i levostranom paraspinalnom propagacijom tumora. Bolesnik je operisan lateralnim ekstrakavitarnim pristupom u položaju tročetvrtinske pronacije i istim pristupom je urađena korporektomija s prednjom stabilizacijom, kao i zadnja transpedikularna stabilizacija. Tumor je potpuno odstranjen uz normalnu poziciju implantata i odličan neurološki oporavak bolesnika. Zaključak. Lateralni ekstrakavitarni pristup omogućava sigurnu ventralnu dekompresiju, prednju i zadnju instrumentalnu stabilizaciju kroz isti rez, u istom aktu operacije, bez promene položaja bolesnika tokom intervencije. Položaj bolesnika omogućava odličnu vizuelizaciju hirurškog polja i duralne vreće.
