Epidemiološke odlike i kliničke manifestacije infekcije hepatitisom E u Bugarskoj - izveštaj o 20 bolesnika

Sažetak

Uvod Hepatitis E je jedna od vodećih kliničkih manifestacija akutnog virusnog hepatitisa u zemljama u razvoju. U industrijski razvijenim državama tokom poslednjih nekoliko godina povećan je broj sporadičnih 'autohtonih' slučajeva infekcije virusom hepatitisa E (HEV). Cilj rada Cilj ove studije je bio da se analiziraju epidemiološke, kliničke i laboratorijske osobine infekcije HEV-om među bolesnicima hospitalizovanim na Odeljenju za infektivne bolesti Vojnomedicinske akademije u Sofiji, u Bugarskoj. Metode rada Retrospektivna studija je izvedena između decembra 2004. i septembra 2012. godine na Odeljenju za infektivne bolesti Vojnomedicinske akademije u Sofiji, a obuhvatila je 806 slučajeva akutnog virusnog hepatitisa. Etiološka analiza uspostavljena je pomoću testa ELISA. Statistička analiza urađena je pomoću programâ Excel 2007 (Microsoft, Redmond, Vašington, SAD) i SPSS Statistics 19.0 (IBM Corp., Armank, Njujork, SAD). Rezultati Specifična reakcija na antitela anti-HEV-IgM i anti-HEV-IgG uočena je kod 20 bolesnika (2,48%). Najčešći simptomi bili su žutica (kod 85% bolesnika), zamor (85%), anoreksija (65%), nelagodnost u trbuhu (55%) i povišena telesna temperatura (40%). Srednja vrednost nivoa aspartat-transaminaze bila je 521 IU/l, a alanin-transaminaze 881 IU/l. Holestaza je bila blaga, obeležena srednjim vrednostima gama-glutamil transferaze od 418 IU/l, i alkalne fosfataze od 486 IU/l. Zaključak Izveštaj je obuhvatio 20 autohtonih, sporadičnih slučajeva akutne infekcije HEV-om. Razmatrali smo zoonotsku etiologija virusa i prenošenje infekcije hranom. Ustanovili smo da su životna dob i postojeće osnovne bolesti faktori rizika za teži tok infekcije ovim virusom.
