Pleuropulmonalna ispoljavanja sistemskih autoimunih oboljenja - analiza serije od 84 slučaja

Sažetak
Uvod Sistemske autoimune bolesti mogu uzrokovati razne plućne i pleuralne abnormalnosti. Cilj ovog rada je da se prikažu kliničke i radiološke karakteristike serije bolesnika sa sistemskim autoimunim bolestima hospitalizovanih u tercijarnoj ustanovi. Metode U ovoj retrospektivnoj studiji pregledali smo kliničke i radiološke nalaze kod bolesnika sa dijagnozom sistemskih autoimunih bolesti na Univerzitetskoj bolnici za plućne bolesti tokom devetogodišnjeg perioda. Rezultati Grupa od 84 bolesnika (srednja starost 53,8 godina) sastojala se od 64 žene i 20 muškaraca. Pedeset osam od 84 bolesnika (69,04%) bolovalo je od kolagene vaskularne bolesti (KVB), a njih 26 je imalo sistemske vaskulitise. Dominantan simptom je bio zamor (75,8% kod KVB i 69,2% kod vaskulitisa). Kašalj, hemoptizije i povišena temperatura bili su češći kod bolesnika sa vaskulitisom. Fibroza je bila najčešće radiološko ispoljavanje KVB (26/58), zatim pleuralni izlivi (18/58) i konsolidacije (10/58). Nepravilne konsolidacije su bile dominantan radiološki nalaz kod vaskulitisa (10/26) i praćene su nodularnim promenama (8/26). Histološka potvrda sistemske autoimune bolesti je dobijena kod 28,6% bolesnika, kod 58/84 bolesnika dijagnoza je zasnovana na pozitivnim serološkim testovima i kliničko-radiološkim ispoljavanjima, u dva slučaja na kliničkim i radiološkim karakteristikama prema definisanim kriterijumima. Zaključak Pleuropulmonalna ispoljavanja sistemskih autoimunih bolesti obično se javljaju u šestoj deceniji, pretežno kod žena. Klinički nalaz i pozitivni serološki testovi ukazuju na sistemsku autoimunu bolest. Fibroza je najčešći radiološki nalaz, koji se nalazi kod skoro polovine bolesnika sa kolagenim vaskularnim bolestima, a nepravilne konsolidacije su najčešći nalazi u vaskulitisu.
